Medford Public Schools would like to invite parents with children ready to enter kindergarten (five years old by August 31, 2016) to complete registration at The Parent Information Center, Medford High School during the weeks of April 4, 11, and April 25, 2016. The Parent Information Center hours are:
• Monday-Friday 7:30-12:00 PM and 1:00-2:00 PM
• Extended Hours: Tuesday, April 26, 2:30-6:30 PM
Thursday, April 28 and Tuesday, May 10, 2016 2:30-6:00 PM
The Parent Information Center will be open for registration during April vacation on Tuesday, April 19 and Wednesday, April 20 from 8:00-12:00.

(781) 393-2132 or email firstname.lastname@example.org. It is strongly recommended you register as early as possible. However, registrations will be accepted during regular Parent Information Center business hours until the beginning of the 2016-2017 school year.
Parents or legal guardians must be prepared to bring the following items with them:
Proof of Residency, which includes (2) current utility bills, water bill, mortgage statement or lease in Parent or Legal Guardian’s name or a Notarized Residency Affidavit, which can be accessed at the Parent Information website listed above, plus (2) current utility bills from the person you are living with. Home owner must also sign the affidavit.
Student’s Birth Certificate
Student’s Immunization Record, physical exam within 1 year, lead test results, tuberculosis test results or tuberculosis risk assessment and vision screening with stereopsis. All incoming kindergarten students must be medically cleared before the start of school.
Photo Identification of Parent or Legal Guardian. Legal Guardians will have to provide a Caregiver Authorization Affidavit. (Available at the Parent Center)
For Special Education students: please provide district with Individualized Education Program (IEP).
Non-English or limited English speaking students will be assessed by the ELL staff. If an Interpreter is needed to interpret this information, please call the ELL office at 781-393-2348.
Kindergarten registrations cannot be processed unless all of the required documents (listed above) are received.
All students are assigned to their proximity zone/geographical zone school. Parents who wish to send their child to a school outside their proximity zone may fill out a choice selection form at the time of registration. Choice assignments are granted based on space availability, sibling preference and diversity plan guidelines. Parents who are granted their choice are notified at the end of August. If choice school is approved, transportation is parent’s responsibility.
